Requisites:
Description:	Aims: To update underpinning knowledge of a range of long term conditions and therapeutics enabling pharmacists to support patients in the self-management of their conditions, through optimising medicines use. Learning Outcomes: On successful completion of this unit students will be able to: 1. Critically discuss the pharmacist's role in managing long term conditions. (Medicines Optimisation Tool [MOT]) 2. Critically evaluate the evidence base for the management of cardiovascular disease, specifically hypertension, atrial fibrillation, stroke, transient ischaemic attack and chronic kidney disease (CKD) (intellectual skill) (MOT) 3. Analyse and critically reflect on your role in reviewing medication and monitoring their safety and effectiveness in patients with the above conditions. (knowledge and understanding) (MOT, Clinical Activity Reflective Exercise [CARE], portfolio tasks) 4. Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions. (MOT, portfolio tasks) 5. Apply good communication and patient centred consultation skills to enhance the patient experience. (Case Based Discussion [CBD]) 6. Apply commonly used clinical skills relevant to the above conditions i.e. blood pressure and pulse measurement. (taught / facilitated) 7.
