Description: | Aims: The aim of this unit is develop deeper clinical knowledge and more sophisticated professional skills in pharmacy practice through studying a range of scenarios focused on specific, complex and co-morbid disease states/conditions. Learning Outcomes: * Practise the advanced skills and attitudes needed to provide holistic patient care in clinical pharmacy; * Relate aspects of prior knowledge from years 1 to 3 to the developing pharmaco-therapeutic knowledge gained in this module; * Solve complex patient care problems by describing the causes and be able to defend and critically justify the selected treatment chosen to peers and members of the multidisciplinary healthcare team * Acknowledge the limits of your knowledge, and refer cases on to other members of the multi-professional team as appropriate, through reflection on your experiences in ward-based learning and inter-professional learning; * Critically evaluate current treatment guidelines and then develop, design and manage a treatment plan for medicines and medicines review; * Further develop and practise the interpersonal communication skills necessary to interview a patient; elicit information from several healthcare sources and prepare a pharmaceutical care plan * Demonstrate the qualities and transferable skills necessary for practising as a pharmacist including the exercise of initiative and personal responsibility, decision-making in complex situations and maintaining your continuing professional development. Content: Topics selected from the following: * Prescribing: Communication skills and physical examination skills * Cardio Vascular (HF and ACS) * Care of the Elderly * Coagulation * COPD * Diabetes * Psychosis * Stroke * Surgery. |
