| Description:
| Aims: To update underpinning knowledge of key long term conditions and therapeutics enabling pharmacists to support patients in the self-management of their conditions through optimising medicines use. Learning Outcomes: On successful completion of this unit students will be able to: 1. Critically evaluate the evidence base for the management of key long term conditions (LTCs), specifically ischaemic heart disease (IHD), acute coronary syndrome (ACS), myocardial infarction (STEMI and NSTEMI), secondary prevention of IHD, chronic heart failure, lipid modification in primary and secondary cardiovascular disease (CVD risk reduction), peripheral arterial disease (PAD) and depression associated with LTCs. (Intellectual skill) (Medicines Optimisation Tool [MOT], portfolio task) 2. Reflect on your role in reviewing medication and monitoring their safety and effectiveness in patients with the above conditions. (knowledge and understanding)(MOT, Clinical Activity Reflective Exercise [CARE], portfolio tasks) 3. Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions. (MOT, portfolio tasks) 4. Apply good communication and patient centred consultation skills to enhance the patient experience. (Case Based Discussion [CBD]) 5. Apply relevant clinical tools and interpret investigations to analyse risk and monitor patients with the above conditions. (taught / facilitated) 6. Safely review medication and optimise medicines use in patients with the above conditions.
