* Critically discuss the pharmacist's role in managing long term conditions.� � * Critically evaluate the evidence base and apply clinical reasoning and professional judgement to the evidence base� for the management of� key long term conditions (LTCs), specifically asthma, chronic obstructive pulmonary disease (COPD), diabetes and hypothyroidism.� * Analyse� and critically reflect on your role in reviewing medication and monitoring their safety and effectiveness in patients with the above conditions.� * Explain the pathophysiology, symptoms and patient monitoring factors in the above conditions.� * Apply good communication and patient� centred� consultation skills to enhance the patient experience� and promote shared decision-making.� � * Apply commonly used clinical skills relevant to the above conditions� e.g.� spirometry, peak flow, HbA1c and diabetic foot examination.� * Safely review medication and� optimise� medicines use in patients with the above� conditions.� |
