Once you've completed the form, you should submit it to the relevant HR Team, attaching scanned copies of receipts as evidence of payment:

• your HR Recruitment Co-ordinator for new visa applications
• the Staff Immigration Team (hr-visas@bath.ac.uk) for visa extensions and Indefinite Leave to Remain applications
